Study on the Diagnostic Stability of DSM-IV-TR Brief Psychotic Disorders in Koreans

Abstract

|
Background and Objectives£ºBrief psychosis is not a sharply demarcated and little is known its nosological entity.
Previous studies indicate that 30% to 60% of patients with acute transient psychotic disorder (ATPD) are rediagnosed with bipolar disorder or schizophrenia but there is no way to expect who will be re-diagnosed as other disorder. The aim of this study was to evaluate the diagnostic stability of DSM-IV-TR recurrent brief psychotic disorder (BPD) retrospectively.
Method£ºWe investigated 30 patients who had a first-ever admission as DSM-IVTR BPD and were re-admitted at least once to the psychiatric ward of the Asan Medical Center from 1989 to 2009. An analysis of re-admission pattern of all subjects was conducted.
Results£ºOnly 10 cases retain their diagnosis on subsequent admission and diagnostic stability is 33.3%. The rest, 20 cases are re-diagnosed at the last£»11 for bipovlar disorder, 4 for schizophrenia, 4 for schizoaffective disorder and 1 for schizophreniform disorder. No significant demographic, social or clinical predictors were found.
Conclusion£ºThe findings imply the need for validation of the concept of BPD. We suggest that BPD have some similarities with bipolar spectrum disorder.
